For each person, we complete a thorough assessment of needs, and develop a personalised care package which focuses on supporting the person to achieve their individual goals.

The care package will be developed around the person and will address their cognitive, behavioural and functional needs as well as highlighting areas that require additional support. We aim to provide guided leisure and social time where required and support community access in a way that meets the individual needs of the person in line with their funded hours.

Our care team regularly review support plans and involve interested parties which may include family, the referring funding authority, and other professionals to discuss an individual’s progress and agree next steps. We continue to actively work on an individual’s goals identified in the assessment and making new goals as the individual progresses and changes.
